1. Data Controller

2. Data We Collect
The appointment form collects your full name, phone number, selected service and, only if you provide it, your email address. The form does not contain a free-text message field and is not intended for sending health data. Medical records are created separately while dental care is provided and in accordance with the regulations governing healthcare.
3. Purpose of Processing
We use the data from the form solely to process your request and contact you to arrange an appointment. The request is sent by email to the clinic at info@smile-design.rs, with a technical copy sent to the website maintenance provider at office@weboperater.com for delivery monitoring and troubleshooting. Google reCAPTCHA is used to protect the form from misuse. We do not sell form data or use it for advertising.
4. Data Retention
Request data is retained while the request is being processed and thereafter only to the extent necessary to fulfil the stated purpose, resolve technical issues or comply with legal obligations. Retention periods for email inboxes and medical records are governed separately according to their purpose and the applicable regulations.
5. Your Rights
You have the right to request access to, rectification or erasure of your data, or restriction of its processing, and to withdraw consent where processing is based on consent. You may submit a request to info@smile-design.rs.You also have the right to contact the Commissioner for Information of Public Importance and Personal Data Protection.
6. Essential and Optional Technologies
Essential technologies include the form's security session, storage of the selected light or dark theme, storage of your privacy choices and protective network features. Google reCAPTCHA loads only when the form is submitted and may process the technical data needed to assess misuse in accordance with Google's policies.
We use Google Tag Manager to load Google Analytics 4 in a controlled manner. Analytics is disabled by default: before you give consent, Google Tag Manager is not loaded and analytics and advertising consent parameters are set to denied. If you accept optional content, Google Tag Manager and Google Analytics 4 are loaded, while advertising purposes remain disabled.
Your choice is stored only in your browser for up to 180 days and can be changed at any time using the ‘Change consent’ option in the footer. 7. External Service Providers
The website may involve hosting and email providers, the website maintenance provider, Cloudflare for network protection and technical telemetry, and Google for reCAPTCHA, optional analytics and maps. Each provider receives only the data required for the specific technical purpose.
